The Evolving Therapeutic Landscape of Diabetes Mellitus and Obesity: From Conventional Treatments to Next-Generation Metabolic Therapies

Abstract
Diabetes mellitus and obesity have converged into a single, interlinked metabolic epidemic that now touches almost every health system in the world. For decades, pharmacological management of type 2 diabetes mellitus centred on glycaemic control alone, with obesity addressed separately through lifestyle counselling or bariatric referral. This convergence has now been overturned by a wave of therapeutic innovation built on incretin physiology, sodium-glucose cotransporter biology and multi-receptor agonism, which treats adiposity and dysglycaemia as a shared pathophysiological target. This review traces the trajectory of metabolic therapeutics from biguanides, sulfonylureas and insulin through dipeptidyl peptidase-4 inhibitors and sodium-glucose cotransporter-2 inhibitors, to the current generation of glucagon-like peptide-1 receptor agonists, dual incretin co-agonists, triple hormone receptor agonists, amylin analogues and oral non-peptide agents. Bariatric and metabolic surgery, gut microbiome modulation and precision medicine approaches are examined alongside pharmacotherapy as complementary strands of an increasingly integrated treatment algorithm. Comparative efficacy, cardiometabolic and renal outcomes, safety profiles and practical implementation issues are synthesised from the peer-reviewed literature, with attention paid to gaps that persist around long-term durability, equity of access and combination-therapy sequencing. The review concludes that metabolic disease management is shifting from single-target glucose lowering towards multi-hormonal, weight-centred therapy, a transition with substantial implications for clinical guidelines, health economics and future drug development.
